If you are interested in applying for a Saint Francis Service Dog, please click on the link below to download the Service Dog Application or call the Saint Francis office at 540-342-3647 to have one mailed to you. Mail your completed form with the application fee to Saint Francis. Once Saint Francis receives your completed application for a service dog, you will be contacted to begin the screening process.

If you are a veteran and are interested in applying for a Saint Francis Veteran Dog, please click on the links below to download the Service Dog Application and Medical Authorization Form or call the Saint Francis office at 540-342-3647 to have them mailed to you. Mail your completed forms with the application fee to Saint Francis. Once Saint Francis receives your completed application for a service dog, you will be contacted to begin the screening process. *Note: We suggest that you also contact your local VA rehabilitaton specialist to ensure that you receive all the services you are entitled to and that there is a coordination of services.

Like most service dog organizations, Saint Francis maintains a waiting list. The sooner you successfully complete the screening and orientation process, the more quickly you can be considered for a Saint Francis Service Dog.

Please Note: Currently Saint Francis Service Dogs is not accepting new applications for service or facility dogs. If you are interesting in being notified when applications are being accepted again, please fill out this Contact Information Form and submit it.
