Saint Francis of Assisi Service Dog Foundation is dedicated to making individuals’ lives more independent through the use of dedicated service dogs.

At this time, the waiting list for a Saint Francis Service Dog is full and no applications are being accepted. As Saint Francis Service Dogs are placed, applications will be accepted.

If you are interested in a Saint Francis Service Dog, please download a Contact Information Sheet or call the Saint Francis office at 540-342-3647. Click the paw prints to download a one-page Contact Information Sheet.  Mail or fax your completed form to Saint Francis.  You will be contacted by Saint Francis and invited to submit an application when there is room on our waiting list for another candidate.  There is no fee or obligation for submitting a Contact Information Sheet.

When your Contact Information Sheet is received, you will receive written confirmation from Saint Francis.  Periodically, we will update you on the status of our waiting list.  We encourage you to contact Saint Francis with any questions or to alert us to changes in your contact information.  Most service dog organizations have a waiting list and the demand for service dogs is growing.  The wait for a Saint Francis Service Dog can be very difficult, but those who have been through the process tell us that it is worth the wait.
